What is Hormone Replacement Therapy?

Hormone replacement therapy (HRT) helps alleviate unpleasant symptoms associated with menopause by supplementing declining hormones. As women approach menopause, their ovaries produce less estrogen, progesterone, and testosterone. 

This hormone fluctuation is responsible for hot flashes, night sweats, vaginal dryness, mood changes, and sleep disturbances. 

There are two main classic HRT regimens:

Historic approach (ie Old Way):

In 2002-2004 studies (WHI) incorrectly suggested that Estrogen and progesterone may be harmful to women’s health. This opened our eyes to re-evaluating the cause of menopause and other treatment options.

The WHI studies were likely the greatest detriment to women’s health in modern era, not because of a flawed study, but due to a misunderstanding of the outcomes. Enter testosterone. Women have 20 times more testosterone than estrogen (but about 1/10th as much as men). And over the next decade we discovered that testosterone reduced menopausal symptoms on the same order as estrogen alone, namely due to the fact that estrogen is a byproduct of testosterone.

Moreover, since testosterone was FDA approved for women with metastatic breast cancer in the 1950’s, we found that testosterone supplementation not only relieved symptoms of menopause, but also substantially protected women against breast cancer.

Later evidence showed that estrogen is effective for the vasomotor symptoms of menopause, bone health, healthy skin, healthy weight, sexuality, and sleep, but also is protective of the cardiovascular system.

This has led to a more contemporaneous approach to menopause, replacing the hormones in a bid-identical fashion.

Half the women took HRT (estrogen with or without progestogen) while the other half took none. After the full course of study, the HRT group still had a 39% lower risk.

It is controversial whether estrogen can increase the risk of breast cancer. In the largest study to date (WHI trial), the estrogen only group had a slightly less risk of breast cancer than untreated menopausal women. This has led us to a better understanding of how to not only help with menopausal symptoms relief, cardiovascular protection, bone and brain protection, but also breast cancer protection with the addition of testosterone. Testosterone appears to protect against breast cancer with or without the addition of estrogen. It doesn’t absolutely protect, but likely reduces the risk of breast cancer by 40-70%.
